Students requesting accommodations in Housing and/or for the academic setting for Fall 2027 should complete the steps below. SSD will schedule appointments to review Housing accommodation requests beginning March 1, 2027. SSD will schedule appointments to review academic accommodation requests June 1, 2027:
- Complete SSD Application for Services.
- Provide documentation of their disability from an appropriate, licensed professional. The documentation must include the diagnosed condition, the treatment being provided (if any) and it can also suggest accommodations. Licensed professionals can use the form below to provide this information. Documentation can be faxed to (402) 472-0080 or emailed to ssddocumentation@unl.edu.
For accessibility purposes, all documentation must be submitted in PDF format as an attachment to email.
