Students who seek accommodations at the University of Nebraska – Lincoln must
- Complete SSD Application for Services. (https://go.unl.edu/ssdapplication)
- Provide documentation of their disability from an appropriate, licensed professional. The documentation must include the diagnosed condition, the treatment being provided (if any) and it can also suggest accommodations. Below are forms that licensed professionals can use to provide this information:
- Attention Deficit Disorders Form
https://www.unl.edu/ssd/PDFs/2022-SSD-ADD.pdf - Chronic Health Disabilities Form
https://www.unl.edu/ssd/PDFs/2022-SSD-CHD.pdf - Mental Health Disabilities Form
https://www.unl.edu/ssd/PDFs/2022-SSD-MH.pdf
- If the student is applying for specific room accommodations, their medical provider should complete and submit the Medical Verification for Housing Accommodations Form IN ADDITION TO one of the preceding forms.

A medical or psychological diagnosis alone does not guarantee housing accommodations. Recommendations from healthcare providers are considered but do not determine eligibility.
Housing accommodations address significant barriers preventing a student from accessing housing equitably. Not all conditions qualify for a housing accommodation.
Single bedroom:
A single bedroom is provided as a disability accommodation only when professional documentation demonstrated a significant access barrier requiring a private sleeping space. For example, medical equipment or conditions requiring specialized living arrangements may qualify.
